We have developed a series of activity sheets to support people with long-term conditions and their carers and family members with self-managing their condition. We will post a new activity sheet every one to two weeks.
These activity sheets compliment MyLifeTool. Each sheet outlines a short, creative activity that encourages you to reflect on a different aspect of self-management, like how you can communicate your needs to others or how you can pace yourself. You can include the activity sheets and what you create in your MyLifeTool toolbox.
